Student Service Consent and Release Form

1. Acknowledgment of Student Services and Supervision
I, the undersigned, voluntarily choose to receive services provided by student practitioners of G.A. Beauty and Barber School. I understand and acknowledge that:

• The services I receive are provided as part of an educational training program where the practitioners are students under the supervision of a licensed beauty or barber instructor.

• A licensed professional, authorized by the Texas Department of Licensing and Regulation (TDLR), oversees the training program; however, due to the nature of the educational setting, a licensed supervisor may not be continuously present throughout every aspect of the service.

• The service providers are students and are not fully licensed professionals.

2. Assumption of Risk
I recognize that receiving services from student practitioners may involve certain inherent risks, including variations in technique and experience compared to those provided by fully licensed professionals. I hereby assume all risks associated with receiving such services.

3. Waiver and Release of Liability
By signing this form, I agree to the following:

• I voluntarily waive any right to bring legal action or pursue legal claims against G.A. Beauty and Barber School, its instructors, employees, agents, and representatives, including the supervising licensed professional(s) as defined under the regulations of the Texas Department of Licensing and Regulation, and in accordance with applicable laws of the State of Texas and Hidalgo County.

• I hereby release and forever discharge G.A. Beauty and Barber School and its affiliated persons from any liability, claims, or demands for damages or injuries that may arise out of or in connection with the services provided by student practitioners, regardless of any alleged negligence on the part of the School or its agents.

4. Voluntary Participation
I attest that I have read and fully understand the nature of the services being provided under this training program, including the conditions of supervision and the associated risks. I voluntarily choose to participate and confirm that I had the opportunity to ask any questions regarding the services and the involvement of student practitioners.

5. Compliance with Regulatory Requirements
This consent and release form is executed in accordance with the regulations established by the Texas Department of Licensing and Regulation (TDLR), the laws of the State of Texas, and applicable laws within Hidalgo County.
